Options for pre-paid dinners and gifts of wine are found on our Credit Card Authorization Form. The form is accepted via fax only and can be downloaded in Adobe Acrobat format onto most computers for printing. If you find that you cannot download the form, please contact us directly at the restaurant; we would be glad to fax you a copy. We ask that you carefully read the instructions on the form, and include the identification required. Forms received without identification will not be processed.
